Welcome to Mount Baker! If you are relocating to Cranbrook and will be attending our school, please complete the forms listed below.
If you are unable to bring the registration into the counselling office, please scan and email the forms to sarah.barnett@sd5.bc.ca.
**Birth Certificate/Passport and BC Care Card, as well as proof of residency (for parent/guardian) MUST accompany Registration**

For new students only - copy of birth certificate, BC care card and proof of BC residency (ie. parent's Driver's License, Rental Agreement, Utility Bill) MUST accompany registration form.

Bus Registration: Please visit the SD5 website for more information about Bus Registration. Bus Registration form must be handed to the BUS DRIVER, not returned to the school.

**Fill out if your student has a life-threatening allergy or medical condition. A doctor MUST sign the Request for Administration of Prescription Medication at School for the form to be valid**
