student-group.jpgWelcome to Mount Baker!  If you are relocating to Cranbrook and will be attending our school, please complete the forms listed below. If you are unable to bring the registration into the counselling office, please scan and email the forms to sarah.barnett@sd5.bc.ca.

**Birth Certificate/Passport and BC Care Card, as well as proof of residency (for parent/guardian) MUST accompany Registration**

AP 316 - Medical Alert      
**Fill out if your student has a life-threatening allergy or medical condition.  A doctor MUST sign the Request for Administration of Prescription Medication at School for the form to be valid**
